Colac Otway Shire is proud to be an approved provider for the Home Care Packages Program. Your home care support requirements will be developed with you in collaboration with our experienced, Colac-based Care Managers. This includes service times tailored to your needs so you receive best value from your HCP budget. You will receive a copy of your individual care plan. We will work together with you, and your carer/family if requested, for positive outcomes.

Home Care Packages Program

The Home Care Package (HCP) Program helps you to live independently in your home for as long as you can. There are four package levels available which is determined by an assessment service through My Aged Care.

Level 1 supports people with basic care needs

Level 2 supports people with low-level care needs

Level 3 supports people with intermediate care needs

Level 4 supports people with high-level care needs.

Each level of home care package provides a different subsidy amount, determined and paid by the Commonwealth Government. (Find out more about other government subsidies available by contacting My Aged Care). This amount is paid to an approved home care provider which you select. The subsidy contributes to the total cost of delivering care to meet your individual needs. As per government guidelines, you may be required to contribute to the cost of your care where your personal financial circumstances allow.
